Upon completion of your purchase, you will receive a document available for immediate download that will contain a link to the detailed questionnaire that you will be required to complete prior to us building your plan. Please note that it is mandatory that you have a gmail account in order to complete the questionnaire. If you purchase the face to face consultation option, you will receive an additional document with instructions on how to schedule your consultation.
